Please note that the GCAAM requires the following fees within two weeks of submitting this form in order to move forward. The initial, required, non-refundable fees are:

  • $300 for Small Claims Mediation (under $30,000 in dispute)
  • $300 for Small Claims Arbitration (under $30,000 in dispute)
  • $500 for Mediation
  • $1500 per party for Arbitration

Failure to pay this amount with void any demand or request which you made.
